ASEAN Notification Of Cosmetic Product

FDA Philippines is accepting online submission of ASEAN Cosmetic Product Notification. You just need to follow the instructions carefully as mistakes may lead to rejected application. 

The following are the four steps to your application:

1. Download the ACD_Form.pdf and the Ingredient.csv
2. Send the duly accomplished form and template via Email to aseannotification@fda.gov.ph and wait for the Billing Form to be replied to the email address indicated in the submitted form.
3. Pay the Billing Form to any Landbank Branch (pending for MOU approval) or to the FDA Cashier and wait for the Result of Application. 
4. If approved, download, print, sign, and affix company logo to the ASEAN Notification form.
