DDB ADVISORY:Yellow Prescription Pads Now Available

The Department of Health announced that the Yellow Pad are now available for distribution. They also would like to inform all pharmacies, physicians, dentists and veterinarians that the ordinary prescription pad can be used in prescribing dangerous drug until June 30, 2012 or until DOH Special Prescription Form is ready.

The ordinary prescription form is valid only if  provided in three copies and with the details stated below. Original copy will be surrendered to the pharmacy, the duplicate is retained to the patient, and the third one is the prescribing physician's copy.

1. Date of Prescription
2. Name and Address of the Patient
3. Name, address and contact number of the prescribing physician, dentist, or veterinarian with the            following:
  • The Professional Tax Receipt (PTR) Number
  • S-2 License Number
  • Professional regulation Commission (PRC) License Number
4. Brand nam or generic name of the dangerous drugs
5. Total number of units to be supplied in words followed by its equivalent in numerals enclosed in parenthesis
6. Specific direction for use
7. Signature of the physician
